To give OGS the most useful information about your HBITS contractor, you will be asked to answer the following questions on the review form:
  1. Did the consultant meet the qualifications of the position?
  2. Did the consultant perform the work you required in a timely fashion? 
  3. Did the consultant keep you informed of progress and any issues in a timely manner?
  4. Did the consultant comply with work rules and policies?  
  5. Was the consultant respectful of employees and property? 
  6. Was the consultant’s time and attendance satisfactory?
